Job Summary/General Purpose of Job
Assist in preparing clients for possible release from detention and provide ongoing support post-release. For clients who are detained during the pendency of their immigration legal proceedings, the Mental Health Case Manager will work to ensure that all mental health issues are addressed. For clients released from detention during the pendency of their immigration proceedings, ongoing support from the case manager may include accompaniment, emotional support, and other relevant services.
Essential Job Functions and Responsibilities
-Participate, as directed, in the screening of new detained clients.
-Create individualized client release plans designed to help the client meet specific needs upon release from immigration detention and coordinate client pick-up upon release from immigration detention.
-Provide information, referrals, and facilitates linkages for clients to mental health treatment, temporary and permanent housing and public assistance programs.
-Provide on-going supportive and/or case management functions in accordance with the problems and needs of each client in order to help the clients to achieve success upon release. Provide on-going support and case management expertise to homeless persons.
-Advocate for clients for public entitlements and housing opportunities.
-Conduct crisis intervention as needed.
-Document all service contacts on a timely basis including face-to-face client meetings, collateral and networking contacts, and correspondence, and maintain the case notes up to date.
-Performs other related duties as required.
Required Education, Qualifications, Experience
-Possession of a Bachelor’s Degree from an accredited college or university in relevant field (Psychology, Social Work, Health, Nursing, Counseling) and 2 years relevant work experience, or 5 years’ work experience and training which indicates the possession of knowledge, skills, and abilities necessary to perform essential job duties.
-Experience in working with health care and mental health regulatory agencies.
-This position requires the ability to travel to the Otay Mesa Detention Center approximately 3-4 days per week. This position requires travel to immigration agency appointments for court hearing dates, check-in appointments with Immigration & Customs Enforcement, and case appointments with US Citizenship & Immigration Services. In accordance with the federal regulation, background check will be required for this position.
-Have reliable transportation to travel to off-site work locations.
Preferred Education, Qualifications, Experience
-Experience in social work or working with severely mentally ill (SMI), developmentally disabled (DD), or other special needs populations.
